Answer

For a nursing home, file with the California Department of Public Health, most directly through its Cal Health Find database, where you can search the facility and select "File a Complaint." You can also file a complaint anonymously. If a resident is in imminent danger, CDPH must go on site within 24 hours. For an assisted living facility, contact the Department of Social Services at 1-844-538-8766 or complaints.ccld.dss.ca.gov. You can also call the Long-Term Care Ombudsman CRISISline at 1-800-231-4024, and report suspected physical or sexual abuse to the police.
